CDMRP Pilot Award: Accelerating Early-Stage Innovation and High-Risk Biomedical Discovery

The Congressionally Directed Medical Research Programs (CDMRP) Pilot Award supports exploratory, early-stage, or high-risk/high-impact projects designed to establish feasibility, advance novel concepts, and generate preliminary data needed for future, larger-scale awards. This mechanism was created to encourage bold ideas that may not yet have the depth of evidence required for an Idea Award or Investigator-Initiated Research Award, but possess strong scientific rationale, compelling innovation, and meaningful potential for military health benefit. The Pilot Award enables investigators to evaluate emerging hypotheses, explore new technologies, prototype devices, or validate foundational biological signals that could redefine prevention, diagnosis, or therapeutic practice.

Eligibility typically includes investigators at any career stage and across many disciplines, although some CDMRP programs may restrict eligibility to early-career scientists or first-time applicants. All submissions must be routed through eligible U.S.-based institutions such as universities, nonprofit research institutes, medical centers, or qualified for-profit research entities with strong compliance infrastructure. Each proposed project must directly align with the priorities of a specific CDMRP program—such as oncology, trauma, infectious disease, rehabilitation, neurodegeneration, or toxic exposure—and demonstrate credible potential to improve outcomes for service members, veterans, or military families.

Pilot Award proposals emphasize innovation, feasibility, military relevance, and strategic alignment with program focus areas. Unlike mechanisms that require extensive preliminary data, the Pilot Award places primary value on conceptual strength, sound exploratory methodology, investigator expertise, and a clear justification for how feasibility testing will establish the foundation for future full proposals. Reviewers evaluate scientific merit, novelty, research approach, military relevance, and potential clinical or translational impact. A programmatic review then evaluates broader portfolio balance and fit with CDMRP strategy.

Common project types include feasibility testing for novel diagnostics, foundational biomarker studies, early-stage therapeutic screening, device prototyping, mechanistic research, precision rehabilitation concepts, or emerging computational and AI-based medical technologies. Successful applicants clearly articulate how the anticipated pilot data will strengthen the next stage of funding—whether as an Idea Award, Investigator-Initiated Award, or follow-on DoD clinical mechanism—and how the innovation addresses a measurable gap in military medicine.
