NIH UH2 Exploratory/Planning Cooperative Agreement (Phased Innovation – UH2/UH3)

The NIH UH2 mechanism supports exploratory, pilot, or planning phases of innovative research projects that require substantial NIH programmatic involvement. UH2 awards are commonly paired with a subsequent UH3 implementation phase, forming a phased innovation structure similar to the R61/R33 model but under a cooperative agreement mechanism with closer NIH collaboration.

The UH2 phase is used to:

  • establish feasibility and preliminary data

  • complete planning and protocol development

  • demonstrate proof-of-concept

  • refine interventions, technologies, or methods

  • meet clearly defined milestones required for advancement to UH3

A defining feature of UH2 awards is that they are milestone-driven and cooperative. NIH program staff work closely with investigators in project oversight, milestone assessment, coordination with other sites or consortia, and progression decisions. Advancement to a UH3 phase is not automatic and depends on successful completion of prespecified UH2 milestones.

UH2 mechanisms are commonly used in programs involving:

  • early translational and implementation research

  • multi-site pilot clinical trials or readiness activities

  • technology development or validation studies

  • large collaborative initiatives requiring structured ramp-up phases

  • public–private or multi-institutional partnerships

Typical UH2 characteristics include:

  • project periods often up to two years for the exploratory phase

  • predefined, quantifiable milestones and decision points

  • substantial NIH programmatic engagement and oversight

  • a linked UH3 phase focused on full-scale implementation or expansion

Competitive UH2 applications clearly articulate:

  • specific, measurable milestones tied to transition readiness

  • feasibility data or strong rationale for phased development

  • a well-defined plan for the subsequent UH3 phase

  • collaborative structure and governance appropriate for a cooperative agreement

  • outcome measures and decision criteria for milestone success

Reviewers evaluate significance, innovation, feasibility, investigator capability, strength of milestones, project management, and the appropriateness of cooperative agreement structure. Because UH2/UH3 mechanisms involve shared stewardship with NIH, clarity in roles, decision processes, and milestone criteria is essential.

At ScienceDocs, our team includes former NIH program officers, cooperative-agreement PIs, and implementation scientists experienced with phased cooperative mechanisms such as UH2/UH3. We assist applicants in crafting strong milestone frameworks, aligning phased aims, developing governance and coordination plans, and positioning projects for successful UH3 transition.
